Read the Instructions for the form you are submitting. The Instructions will answer many of your questions and will tell you what other documents you need to submit.
You are not required to use the Corporations Division form, except for the forms marked with an asterisk. Documents marked with an asterisk in the charts below must be submitted on the Corporations Division form, and the old Corporations Division forms for those documents are still acceptable at this time.
All documents must be submitted with a Cover Sheet. The Cover Sheet is a Miscellaneous form.
All forms are in PDF format and are fillable (you can type in them). We recommend that you type the information so the documents are legible.
